Service Agreements & invoice example
With service providers, it's not mandatory, however it is considered good practice, to have a service agreement in place. Some providers, such as allied health, have their own standard agreement that they will ask you to sign.
Service agreements clarify the terms between a service provider and you. These agreements are essential for managing risk and maintaining a harmonious partnership.
A comprehensive service agreement should include:
- What supports are being provided
- The cost of the supports and using an appropriate invoice
- The responsibilities of your service provider
- Your responsibilities
- How long the agreement goes for and how it can be changed
- The dispute resolution process
For an even greater understanding of service agreements consider becoming a Support Plus Platinum member and have access to a ‘Service Agreement Fillable Form’ and an example of a completed Service Agreement. You can upgrade your membership to Platinum by clicking on any of the ‘padlock’ symbols on the left hand menu.
You should be able to claim the cost of a Platinum membership from your NDIS plan.
Helpful Tip
Your service providers invoice should also reflect key information to satisfy claiming the support from your NDIS plan.
Attached is a 'Service Agreement Invoice example'. This example can be used for your service provider's convenience, in case they do not have one that meets your requirements.
