Facing end-of-life planning is never easy, but having a thoughtful list of your preferences can be invaluable when navigating challenging conversations about your wishes for care. This list serves as a guide to help clarify what matters most to you, especially if you ever face a critical diagnosis or need to make your end-of-life intentions known.

With so many options and decisions that arise in these moments, knowing your preferences in advance can provide peace of mind for both you and your loved ones. Use this list to explore the things you feel strongly about, allowing you to communicate your choices clearly.

Consider questions like:

  • Who do you want by your side? Do you envision your final moments surrounded by loved ones, or would you prefer a quieter setting?
  • Do you want music playing? Is there a particular song or type of music that brings you comfort?
  • How do you feel about physical touch? Do you find comfort in being held or touched, or would you prefer privacy?
  • Where would you like to spend your final days? If possible, would you prefer to be at home rather than in a hospital or hospice?
  • What are your views on organ donation? Are you registered as an organ donor, and do you want your family to support that decision?
  • What are your preferences around resuscitation or life support? Would you want measures taken to prolong life, or prefer to decline interventions if recovery isn't possible?
  • What would you like for your body after passing? Do you prefer burial, cremation, or perhaps a green, environmentally friendly option?
  • Do you want a funeral or memorial service? Consider if you would like a public or private service, or perhaps a simple non-service cremation.
  • Are religious elements important to you? If so, would you prefer a church or chapel setting, led by a priest, celebrant, or spiritual leader?

Creating this list and discussing your wishes with those close to you can make an incredibly difficult time a bit easier. By taking the time now to reflect on these questions, you give your loved ones the gift of clarity and the ability to honor your preferences when the time comes.
