Checklist if you are Participant’s new support person &/or nominee
Ideally, a smooth handover of Participant's caregiver arrangements would utilise the information provided in each module. However, if this is not feasible, the following checklist can serve as a helpful guide for the transition process.
- Meet with Participant and go through what you will be helping with. Cover the details outlined in the ‘
About Participant’ module during the discussion. - Change of Nominee – Ring the NDIS on 1800 800 110 and request a change of nominee form.
Email the completed form with a copy of certified 100pt ID to: [email protected] - Email each of Participant’s NDIS providers to advise them that you are Participant’s new contact person. You will find their details in the ‘
Service Provider’ module. The ‘
Resource Library’ module has a suggested email text, ‘Change of Contact Details’ for you to use. - Contact each of Participant’s non-NDIS providers as needed. Refer to the ‘Service Provider’ module for their details.
- Familiarise yourself with the components of Participant’s NDIS plan, using the ‘
Participant’s NDIS Plan’ module for guidance. Make a diary note 8 weeks prior to when the plan is due for reassessment in order to prepare. - Phone Participant’s support coordinator (if applicable), Local Area Coordinator or Early Childhood Partner to advise the change in Participant’s circumstances. Their details should be in the ‘
Service Provider’ module. They are an excellent resource if you have any NDIS related questions. - Familiarise yourself with the NDIS portal, myplace, and set up your login. When your plan nominee has been approved, phone the NDIA on 1800 800 110 and request an NDIS activation code (refer to the
‘
Navigating the NDIS’ module for guidance). - Download the myNDIS mobile app (refer to the ‘
Navigating the NDIS’ module for guidance). - Verify the status of Participant's service provider claims and take appropriate action as needed. Utilise the myNDIS app or myplace portal to review recent payment claims made (refer to the ‘
Navigating the NDIS' module). If Participant is self managed, cross-check Participant's NDIS bank account statement to ensure payments have been correctly disbursed to the respective providers. - Set up an electronic file to store NDIS related documents.
- Obtain a copy of past documentation and store them in your new electronic file. Refer to the 'Who is doing what' section of this module to see where Participant's current documents are currently stored and how to locate them.
Helpful Tip
Whenever you ring the NDIA, make sure you request a receipt number. The NDIA should add the conversation details to Participant’s file along with the receipt number.
This helps with accountability and future tracking of activity.
